A simple, editable, one-page student survey for teachers that gives your students the opportunity to evaluate your course and you as a teacher.
Opening yourself up to feedback and potential criticism can be scary. But I truly believe you and your students will benefit from this experience. They will reflect on their time with you, and you will receive feedback that helps you see your strengths as well as areas for growth.
I always used a student survey at the end of the year because I wanted to continually improve as a teacher. I will admit that I got my feelings hurt a few times, but the positive feedback far outweighed the negative! I’m retired from teaching now, but I still have some of those evaluations. They’re a treasure!
Survey Layout
This student survey is divided into two sections. Section 1 is a chart for students to rate 10 areas that are directly related to you, the teacher, and how you conduct your class.
Section 2 features open-ended questions which are more focused on the class itself.

Includes tips for implementing this evaluation including:
- how to edit the questions
- when to use it
- sample instructions to give students
- how to handle negative and positive feedback
